Reply with quote | #1 | I guess we usually aim for steroids and antihistamines. Some go for the night before for steroids if possible. Hmm there is debate about using Omnipaque or Visipaque. Some say either. Both have iodine in them of course so i suspect no difference, I must ask around to see what is the best. __________________ http://www.smarthearts.info

My understanding is that it is not the iodine that is responsible for contrast reactions. Previous known contrast reaction should be the indicator for prophylactic cover. We premedicate with Hydrocortisone prior to subsequent studies and stick with Visipaque, as it is the superior product (IMO). Known seafood allergy is also misleading. It is more that a history of anaphylactic/allergic response to seafood is an indicator that they may have a similar reponse to contrast (along with other compounds). We don't often see contrast reactions in our lab (fingers crossed) and I can only recall one full on anaphylactic response in the last 5-6 years. __________________ Only dead fish go with the flow |

Hey Cath MRT! Great to see you here, great answer too BTW. and good to see you here too Pete. Both of you Veterans of the web blog/post scene. About time we had some proper discussion forums for the cath labs. Will try to pop into these forums more often but the computer is residing in the bedroom at the mo and limits my late nite online contact.

Reply with quote | #7 | Hi All Yet another Australian. We use Hydrocortisone if the person has not been identified prior and there is a need to do the case with some haste. Anti- histamines as well more for the urticarial reaction. Hydrocortisone will take at least 6 hours for effect and is really for subsequent serum sickness post. Prednisolone for 3 days prior is best treatment. Some use ranitidine as it has antihistamine actions also. It is the proteins with shellfish allergy that tends to be the problem rather than iodine. We have visipaque on hand for renal patients rather than for allergic reactions. Iodine allergy is usually a pre-identified issue where a previous contrast reaction has been noted. We have had no anaphylactic reactions here in my 10 years. Mostly urticarial rash.
