Reply with quote | #1 | Was wondering if any other cath labs are bringing in acute stroke patients? A group of our cardiologists (and one neurosurgeon) is putting together a "Code Stroke" protocol to bring in these patients. I know of angio departments in radiology doing this, but the argument here is that the staff in radiology cannot handle the acuity of these patients (!).
I unfortunately don't have many details yet, but am concerned about training all the on-call staff to perform competently in these procedures. As it stands now, we have a core group of only 12-15 people who assist with the carotid stenting.
Any advice/suggestions/comments would be greatly appreciated.

Reply with quote | #2 | Yep! We do Acute Stroke as well (however it is a bit political, and most patients go to neuroradiology). I do have to say, these cases are the most "fun" and you feel like you have a sense of accomplishment when they are successful.
Acute strokes remind me of the "old days" of AMI management, before stents, IIb/IIIa inhibitors, IV Lopressor, etc. Nearly every AMI patient came into the lab in excruciating chestpain, most had received lysis, and you chased labile BPs, managed rhythms, and nausea/vomitting. With "modern" pharmaceutical regimens, It is rare to see a STEMI in that condition these days.
Cleveland Clinic south? Last I heard, we only had medical centers in Ohio, Florida, Toronto, and Abu Dhabi (capital city of the United Arab Emirates).

Reply with quote | #3 | Thanks for the post, Ken. We're all very nervous about bringing in acute stroke patients. How did you train all the staff?

P.S. When you've got 4 docs who trained at Cleveland Clinic, it starts to feel like a satellite campus around here sometimes! |
