I am just interested in what you guys do across the pond when it comes to xray procedures/doses:
  • Do you have an audit on doses in the cath lab?
  • Do you have a high dose reporting (skin dose or DAP) for patients?
  • If you do have a high dose limit, what is it?
  • If they breach this what happens?

It depends on what part of the country you live in.  At the Cleveland Clinic, Radiation Safety decided to be proactive, before reporting becomes mandatory by the state, Joint Commission, and/or Federal Government.  It is being implemented as part of the patient's permanant medical record.

As you may be aware, depending on equipment brand and age, you may or may not have the ability to measure of Dose Area Product (DAP).  

Open the attachment to view our current policy.  We notify the physicians if/when they hit the 80% of the consultation dose.

Does anyone else have a high dose reporting limit, if so what is it?

Ours is 15,000 cGycm2 (followed up in clinic) and 20,000 (reperted to Radiation Protection Advisor). Then RPA does risk assessment.

We are in the process of developing a follow up protocol for high dose procedures as part of a Radiation Safety Policy.  I suggested a threshold of 30,000 cGycm2 as a threshold but our Medical Physics guys suggest 40,000 cGycm2 which I feel is maybe a bit too high.

How is everyone arriving at a figure?  My figure of 30,000 is based on posts on other websites but I would like to have some supporting evidence to give to Med Phys.  Unfortunately they have opted to not be actively involved in developing our Radiation Safety Policy and have left it up to us to develop.  Currently there isn't a Radiation Safety Officer employed at our hospital and I feel we are floundering a bit so would appreciate any input.

Some say a good estimate is the 95th percentile of all your doses.
